Summer Music Camp Liability Waiver & Release of Liability

By registering my child for the Summer Music Camp, I acknowledge and agree to the following:

1. I understand that my child will participate in activities including, but not limited to, music instruction, singing, movement, dance, arts and crafts, storytelling, games, and other supervised recreational activities.

2. I certify that my child is physically able to participate in these activities and that I have disclosed any relevant medical conditions, allergies, medications, or special needs on the registration form.

3. I understand that, while every reasonable precaution will be taken to provide a safe environment and proper supervision, participation in camp activities involves inherent risks of injury that cannot be completely eliminated.

4. I voluntarily assume all risks associated with my child's participation and release, waive, and hold harmless the Summer Music Camp, its organizers, instructors, volunteers, employees, and affiliates from any claims, liabilities, damages, or expenses arising out of my child's participation, except to the extent caused by gross negligence or willful misconduct.

5. In the event of a medical emergency, I authorize the camp staff to obtain emergency medical treatment for my child if I cannot be reached immediately. I understand that I am responsible for any resulting medical expenses.

6. I grant permission for photographs and/or videos of my child taken during camp activities to be used for promotional, educational, or marketing purposes unless I indicate otherwise in writing before the start of camp.

7. I understand that submitting this registration form constitutes my acknowledgment and acceptance of this Liability Waiver and Release of Liability.
